TİP 1 DİYABETİK HASTALARDA EGZERSİZİN GLİSEMİK KONTROLE ETKİSİ: BİR META-ANALİZ ÇALIŞMA

Abstract

Tip 1
diyabet otoimmün nedenlere bağlı olarak pankreasta insülin üreten beta
hücrelerin tahrip edilmesiyle ortaya çıkan ve ciddi sağlık sorunlarına yol açan
süreğen bir hastalıktır. Çocuklarda ve gençlerde görülme sıklığı giderek
artmaktadır. Ancak hastalar insülin tedavisi, yakın izlem, doğru bir diyet ve
düzenli fizik aktiviteyle kan glukoz seviyelerini uygun aralıkta tutarak daha
sağlıklı bir yaşam sürebilirler. Uzun yıllardır literatürde düzenli egzersiz
yapmanın tip 1 diyabetli hastalar üzerindeki etkileri incelenmektedir. Bu
araştırmalarda elde edilen sonuçların etki büyüklüklerini değerlendirmek
amacıyla; araştırmamızda tip 1 diyabetli hastalar için düzenli egzersizin
glisemik kontrole etkilerini inceleyen çalışmaların sonuçlarını meta analiz
yöntemiyle inceledik. Bu doğrultuda PubMed, Scopus ve Web of Science veri
tabanlarında tip 1 diyabet, egzersiz ve glisemik kontrol anahtar kelimeleri
kullanılarak 2007 yılından 2017 yılına kadar yayınlanmış çalışmalar tarandı.
Taramalar sonucunda incelenen 1223 makaleden araştırma kriterlerine uyan ve
meta analiz yapmaya elverişli 9 çalışmaya ait veriler analiz edildi. Bu
çalışmalarda katılımcı sayısı, yaş ortalaması, cinsiyet, uygulanan egzersiz
tipi, haftalık egzersiz sıklığı ve glisemik kontrole ait veriler meta analiz
programı Comprehensive Meta-Analysis Software V.2.2 (Biostat, Englewood, New
Jersey) kullanılarak analiz edildi. Çalışmaların meta analiz sonuçları orta
etki büyüklüğünde düzenli egzersizin kan şekerinin akut ve kronik kontrolü için
olumlu ve anlamlı bir etkiye sahip olduğunu gösterdi (Hedges’ g =0,56,
p< ,05). Sonuçta, tip 1 diyabetik
hastalar için düzenli egzersizin vücutta insülin hassasiyetini ve glikoz
emilimi için uyarılmış kas kütlesini artırarak glisemik kontrol ve genel sağlık
açısından sürdürülebilir bir fayda sağladığı saptandı.
